As such, our acceptance agents are authorized to receive and execute your application for a U.S. passport book and/or passport card. It is also important to understand that only the U.S. Department of State can determine eligibility for a U.S. passport.
This facility is designated to help process your new, lost or stolen, or minor U.S. passport applications using the DS-11 application form only. If you are renewing an active U.S. passport, the U.S. Department of State encourages eligible applicants to renew online or by mail. You can find more information about each renewal method by visiting the . If you decide to renew in person after viewing each requirement, you may schedule an appointment at our facility and bring the form and all required documents. Please note that renewing online or by mail allows you to avoid the $35 acceptance fee that applies to in-person services.
