2. Proof of U.S. Citizenship

  • Original and one B&W photocopy submitted with application
  • Original will be returned to applicant via USPS by U.S. Department of State. Separate from the new passport.

3. Proof of Identification

  • Valid State of Texas ID
  • Color copy of front and back of ID (single-sided)

4. Photo

  • A 2x2 photo with white background, no hats, glasses, etc. More information can be found at .

Note: You may choose to take your passport photo on site at 黑料吃瓜, if you do not already have one. This service is provided in the Passport Services Office.

5. Payment

  • NOTICE: Kindly be aware that the Execution Fee/Acceptance Fee will be paid to 黑料吃瓜 with card only during the appointment.
  • NOTICE: Kindly be aware that the Application Fee will be paid to U.S. Department of State via check or money order. You must bring a check or money order to your appointment.
